Choose the most convenient UHP participating dentist from the customized list of general dentists included with your membership packet. If you need help or have specific questions, simply call the toll-free number on your membership card.

UHP will connect you to a dedicated customer service agent who will be happy to answer any questions you may have on a participating dental provider; such as…

     • Educational background

     • Years of experience

     • Languages spoken

     • Special services such as handicap        access, etc.

Since you start saving money with your first dental appointment, there‘s no reason to delay. As a UHP dental plan member, you’ll receive an annual oral exam and bitewing x-rays at no charge in most areas (see your fee schedule for details). Plus, you’ll save instantly on all other dental procedures. When you make your appointment, please mention the name “DDS-dental directory services”. This is the exclusive dental network used by UHP and is the name that the dental office is most familiar with.

Just present your UHP ID card each time you visit a participating dental office. Your ID card has the UHP name, your personalized information, and the UHP customer service number, which the dental office will use to confirm your eligibility. Once eligibility is confirmed and dental services are rendered to you, you’ll receive the special reduced fee plan rates. The extensive list of reduced fees is also included in the membership packet.
